Before any Comfort IV treatment begins, every client goes through a structured health screening process. This includes a health intake form completed before your appointment and real-time review and approval by a licensed Nurse Practitioner before your IV starts. This is not optional at Comfort IV. It happens at every single appointment.

Why Every Comfort IV Appointment Starts With a Screening

IV therapy delivers nutrients and medications directly into the bloodstream, which means they take effect faster and more completely than oral administration. This is exactly what makes IV therapy effective. It also means that clinical review before treatment is more important than it is for oral supplements.


Pre-treatment screening identifies health conditions, medications, and clinical factors that affect which treatment is appropriate for you, what dose is right, and whether any modification is needed before proceeding. Skipping this step is not a shortcut. It is a safety failure. Comfort IV does not skip it.

Step 1: Your Health Intake Form

Before your provider arrives, you will complete a health intake form covering the information your Nurse Practitioner needs to review your treatment plan. The form is straightforward and takes most clients about three to five minutes to complete.


Your health intake form covers:

  • Current medications and supplements including dosages
  • Known allergies including drug and food allergies
  • Relevant medical history including chronic conditions, recent surgeries, and hospitalizations
  • Current symptoms and the reason for your appointment
  • Pregnancy or breastfeeding status
  • Recent lab work or medical diagnoses relevant to your treatment
  • Your selected Comfort IV package and any requested add-ins

Complete your health intake form as thoroughly and accurately as possible. Incomplete or inaccurate intake information can affect the safety and appropriateness of your treatment.

Step 2: Real-Time Nurse Practitioner Review and Approval

After you submit your health intake form, a licensed Nurse Practitioner reviews your information and your selected treatment plan in real time. This review happens before your provider arrives or before your IV begins. The NP assesses:

  • Whether your selected package and any add-ins are appropriate given your health history
  • Whether any medications in your package interact with your current prescriptions
  • Whether your package needs to be modified based on your clinical situation
  • Whether there are any contraindications that would make treatment inappropriate at this time

If your NP approves your treatment, your provider proceeds with the approved plan. If modification is needed, your NP communicates the change before the appointment begins. If a treatment is not appropriate, your NP will explain why and may recommend you consult your physician before rebooking.

Step 3: On-Site Assessment by Your Provider

When your provider arrives, they conduct a brief clinical check-in before placing the IV. This includes reviewing your health intake information, asking about any changes since you submitted the form, checking your current condition, and confirming your consent to proceed with the approved treatment plan.


If anything has changed since your intake was submitted, your provider will note it and communicate with your NP if needed before proceeding. Your health and safety take priority over schedule.
